MedComm gives hospitals an AI care manager, Heather, that calls and texts heart-failure patients after discharge, follows their clinicians’ protocols, and escalates to a nurse only when needed.
Problem & solution
About 1 in 4 heart-failure patients are readmitted within 30 days, often after problems that could have been caught earlier. Nurses know what to look for, but cannot call every patient consistently. Heather calls and texts each patient for 30 days, follows the hospital’s clinical protocols, checks symptoms, weight and medications, and escalates only when a nurse is needed—so the same care team can follow far more patients.
Primary customers & users
Buyers are hospitals and health systems, plus value-based care organizations such as ACOs and PACE programs. Decision-makers include CMOs/CNOs, population health and care-management leaders, and cardiology service-line leaders. Users are the nurses and care managers who receive Heather’s escalations, and the patients she follows at home.
Differentiation
Most alternatives fall into two groups: broad AI agents that are clinically generic, or heart-failure programs that still depend heavily on people. We combine automation with condition-specific workflows. The moat is not the LLM; it is the clinician-authored protocols, escalation logic, evaluation system and outcomes data built with care teams.
Current traction
Pilots with Weill Cornell in heart-failure transitions and the ED, with Northwell Lenox Hill ED in approval; IntusCare and Ochsner are in scoping. Our first IRB study is complete: 92% of patients reported better understanding and 95% of clinicians endorsed accuracy and efficiency; a paper is in progress. We have run 2,000+ test conversations, raised $95K to date, and received $250K in AWS credits.
